Financial Policy

We deliver the finest care at the most reasonable cost to our patients. For your convenience, we accept cash, check, or card (Visa, MasterCard, Discover and American Express).

With insurance we typically require a minimum of 25% to be paid the day of service for services over $350. Depending on your plan or coverage, a higher amount may be required.

Without insurance, we require 50% to be paid the day of surgery for services over $350.

Typically, charges of $350 or lower require payment in full, whether a patient is with OR without dental insurance. Depending on your dental plan, a lower amount may be required. Balances should be paid within 90 days with monthly payments. If you have questions regarding your account, please contact us at 812-401-3500 or 270-208-9005. In most cases, a simple telephone call will clear any misunderstandings.

Certain patients may also qualify for CareCredit. These patients may have an exam first, where all treatment and financial questions can be answered. CareCredit is the healthcare credit card exclusively designed to finance healthcare services for you or your family. CareCredit offers special financing on purchases of $200 or more for healthcare procedures not commonly covered by insurance, including dental treatments.*

Please remember you are fully responsible for all fees charged by this office, regardless of your insurance coverage. After your appointment, we will send you a monthly statement. Most insurance companies will respond within three to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Once your insurance has paid, the remaining balance is your responsibility. Your prompt remittance is appreciated.
